Maximano Silva, 32, most recently of 77 Huntington Ave., Marlborough, is accused of producing a rubber stamp in the name of a local physician and fake blank prescription forms also in the name of the physician. Silva allegedly generated health insurance cards and credit cards in the names of unknowing persons.
